Official Inquiry: Southport Attacker's Violence Was 'Unambiguously Signposted' but Ignored

A damning official report blames "catastrophic" failures by multiple agencies and the "irresponsible and harmful" actions of his parents for allowing Axel Rudakubana to carry out the Southport attack. The inquiry found the system "completely failed." It stated that some form of violence by Rudakubana had been clearly warned about, or "unambiguously signposted," for many years. Lead investigator Sir Adrian Fulford condemned an "inappropriate merry-go-round" of state bodies avoiding responsibility. He called their refusal to accept blame "frankly depressing" and said this culture must end.
